Process Hazard Analysis (PHA)

BACKGROUND & ABOUT THE COURSE:
Dalam industri, khususnya yang melibatkan proses berbahaya, identifikasi, evaluasi, dan pengendalian risiko potensial sangat penting untuk memastikan keselamatan, kepatuhan lingkungan, dan keberlangsungan operasional. Analisis Bahaya Proses (PHA) berfungsi sebagai pendekatan sistematis untuk mengidentifikasi bahaya, menganalisis potensi konsekuensi, dan menerapkan langkah-langkah untuk mengurangi risiko. Karena proses industri menjadi semakin kompleks, pentingnya kerangka kerja PHA yang kuat telah berkembang, yang membutuhkan personel terampil yang mampu memahami dan menerapkan metodologi tingkat lanjut secara efektif.
Program pelatihan ini dirancang untuk membekali peserta dengan pengetahuan yang komprehensif dan keterampilan praktis dalam melakukan Analisis Bahaya Proses. Dengan berfokus pada teknik-teknik utama seperti Studi Bahaya dan Operabilitas (HAZOP), Analisis What-If, dan Analisis Mode dan Efek Kegagalan (FMEA), program ini bertujuan untuk meningkatkan kemampuan peserta dalam mengantisipasi risiko potensial, mengusulkan langkah-langkah pencegahan, dan memastikan kepatuhan terhadap standar keselamatan internasional.
TRAINING OBJECTIVES:
- Cara melakukan PHA untuk memenuhi persyaratan Process Safety Management (PSM) untuk PHA awal dan manajemen analisis perubahan termasuk elemen-elemen penting berikut
- Cara mempersiapkan PHA; Menentukan cakupan PHA; Mengumpulkan informasi; Memilih tim; Memimpin rapat; Mendokumentasikan hasil
- Bagaimana dan kapan menerapkan metode analisis HAZOP, FMEA, dan/atau what-if/checklist pada sistem proses apa pun di industri apa pun
- Cara menganalisis prosedur operasi untuk skenario kecelakaan kritis
- Menerapkan PHA untuk memastikan operesional & maintenance yang lebih aman

- Introduction: Learning objectives; Overview of process safety management; Risk assessment concepts; Overview of PHA requirements
- Overview of risk review methods: Methods and their usefulness over the life cycle of a process; Making risk judgments; Human factors concepts and how to address human factors during hazard evaluations
- Preparing for the hazard evaluation (risk review or PHA): Scoping the analysis; Choosing technique and level of detail and sections; Choosing the team members; Logistics and procedures for pre-meeting, meeting, and post-meeting tasks
- What-if/checklist technique: Workshop: What-if/checklist review of a continuous process
- HAZOP technique: Workshop: HAZOP review of a continuous process
- Analysis documentation, results, and follow-up: Workshop: HAZOP review of a continuous process, HAZOP/what-if techniques for analyzing procedures and batch processes

ABOUT THE TRAINER:
Ir. In Jumanda Kasdadi, MT (IJK)
In addition to his duties as a lecturer, Ir. Jumanda, MT, has conducted nearly 280 training sessions (public and in-house) across various industries since 1997, particularly in the oil and gas sector. He holds Bachelor’s and Master’s degrees in Chemical Engineering from the Bandung Institute of Technology (ITB) and has completed training in nine oil and gas topics at PETEX (Petroleum Extension), University of Texas at Austin, Houston, Texas, USA, in 2018. With his extensive experience, he has become a prominent expert in industrial engineering training.
Ir. Jumanda also holds numerous professional certifications, including a Lecturer Certificate from Indonesia’s Ministry of Education, certifications in Construction, Electrical, and Fire Protection from HSE Association Indonesia (ALPK3), as well as certifications in Risk-Based Inspection (RBI) Facilitation, HAZID, and HAZOPS from BNSP. He has delivered training to major corporations such as Pertamina, Chevron, ConocoPhillips, Total Indonesie, CNOOC, Medco, Freeport, and SKK Migas, along with international companies in the petrochemical, mining, and energy sectors.
In addition to training, he has been directly involved in various strategic projects, including Feasibility Studies (FS), Front-End Engineering Design (FEED), Detail Engineering Design for Construction (DEDC), and risk analyses such as RBI, HAZID, HAZOPS, QRA, and SIL Classification for national and international companies. His expertise and contributions have significantly impacted the development of the oil, gas, and energy industries in Indonesia and globally.
